Admissions

Whether for short-term rehabilitation or long-term care, the Gurwin Jewish Nursing & Rehabilitation Center is here to provide the highest quality care and caring for our residents.  We are delighted that you are considering Gurwin for your care needs, and will do anything we can to make the process of admissions easier.

The most important paperwork in the admissions process is the Patient Review Instrument (PRI), which is usually completed by a nurse or social worker in the hospital or other nursing facility.  Should the admission be from the community (home or assisted living residence), the PRI may be completed by a physician or nurse in the community.

Once the PRI is completed, please have it faxed to our Admissions office at (631) 715-2905.  At that point, we may begin the process of reviewing the patient for admission.  Additional personal, medical and/or financial information may be required, and a designated representative will be contacted to provide it before admission, if necessary.
