SKILLED NURSING
Whether at our Gurwin Center in Commack or at Island Nursing and Rehab Center in Holtsville, you can be sure of high-quality care by staff who care for you like family.
ADULT DAY CARE
Two programs offer health-related services or memory care support, with a full calendar of activities and nutritious meals and snacks.
HOME CARE
Our two home care programs offer Home Health Aides and Companions and Skilled Services – Gurwin quality care, right in your own home.
MEMORY CARE
Compassionate, dignified memory support is provided for residents in both our skilled facilities as well and our assisted living community.
RESPIRATORY CARE
