There are more than 4,000 adult day care centers in the United States, and the need is growing for this type of service. Gurwin offers both medical and social model adult day care programs to meet the needs of our Long Island community. The medical model adult day health care (ADHC) program provides not only the social activities associated with most senior programs, but also the medical/nursing and rehabilitation services ordered by a physician that will help our registrants remain living in their own homes. The social model offers frail elderly individuals and those with Alzheimer’s disease or other dementia a structured, active, friendly and supervised environment where they can flourish.

Health and recreation services are provided in a caring and creative environment, designed for the care, comfort and convenience of the frail, elderly and younger disabled adult.

Gurwin Adult Day Health Program Medical Model

Certified by the New York State Department of Health, Gurwin’s medical model program operates five days a week, from 8:30 am until 3:00 pm.  Flex hours are also available. It is designed for elderly and younger adults who:

  • Have cognitive, physical or psychological conditions requiring supervision including: Alzheimer’s disease or other related dementias, coronary artery disease, diabetes, Parkinson’s disease, stroke, osteoarthritis, Multiple Sclerosis, Muscular Dystrophy, head trauma, etc.
  • Require health and/or rehabilitative services.
  • Require a structured program of emotional and social support and stimulation.

Gurwin Adult Day Program Social Model

Our social model program operates Monday through Friday from 8:30 am until 3:30 pm (4 hour half-day rates also available) and is designed for the frail elderly and those with Alzheimer’s disease and other dementias.  Private pay, Medicaid Managed Care and insurance plans are accepted.   The program is run by our compassionate, caring bi-lingual staff and includes:

  • Two meals and snacks
  • A variety of stimulating activities including art exploration, crafts, exercise, games, dancing, gardening, trips, holiday and birthday celebrations
  • Yoga and meditation, aromatherapy and more
  • Assistance with Activities of Daily Living
  • Cognitive stimulation including reminiscence, games and puzzles

Adult Day Health Care Community

Both of our Adult Day Health Programs are located in the Gurwin Jewish Nursing & Rehabilitation Center, with a private entrance and facilities specifically designated for our program participants. Access to clinic services, a beauty salon/barber shop,  and outdoor patios is provided, enabling our program participants to handle routine needs while at the Center, and minimizing those needs when at home.  Our award-winning Therapeutic Recreation program offers a full social calendar with choice, allowing participants to be busy and engaged all day long.

Do you administer medications and do you assist in personal hygiene such as baths, shaving and oral hygiene?

We do perform these services in our Medical Model. Our full-time nursing staff is available for all health care monitoring and attention, including medication administration and other treatments. Personal Care such as grooming, bathing, shaving and hair care can be provided as part of the health care program.

Are there any age restrictions, or cognitive requirements for participation in the program?

Our Adult Day Health Program is designed for adults 20 years old and older. Our staff is equipped to assist registrants with a wide range of needs. Specific group programs are planned daily for those with special needs or diagnoses, such as dementia, Alzheimer’s and cognitive disabilities.
