Gurwin's skilled nursing community is not your traditional nursing home. Our full service, not-for-profit community and award-winning programs provide the highest level of nursing health care on Long Island.

- Rehabilitation

- Gurwin's Short-Term Rehabilitation programs are focused on helping our patients get back home as quickly as possible. Physical, Occupational and Speech Therapies are provided in state-of-the-art rehabilitation gyms, supported by the care and caring of our nursing and medical professionals.

- Adult Day Health

- Gurwin's Adult Day Health Program provides nursing, rehabilitation, and other health services in a social and exciting atmosphere to adults of all ages. Door-through-door transportation is provided, as well as caregiver respite and support.

- Dialysis

- Working with Island Rehabilitation, Gurwin provides on-site dialysis for participants in any of our programs. This 12-station dialysis center is also available for residents of the community.

- The Gurwin Jewish ~ Fay J. Lindner Residences is Long Island’s premier assisted living community. We offer an unmatched level of care, superior amenities and exciting activities to keep our residents safe, comfortable and engaged. Memory Care is provided in our secure Enriched Pathways community.

- Gurwin offers two home care programs providing personal care and companion services, as well as physician-ordered skilled services such as nursing care and rehabilitation.

Fountaingate Gardens is an Independent Living community for active adults age 62 and over. The addition of this beautiful new community is the final component of Gurwin's continuum of care, creating a Life Plan Community on Gurwin's 36-acre campus.
