Skilled Services

Our Gurwin Long Term Home Health Care Program, also known as a Lombardi Program, provides skilled services as prescribed by a doctor, usually billable through Medicare or Medicaid. 

Rehabilitation Services include Physical, Occupational and Speech Therapies, and are provided in the home by licensed therapy providers.  Often, a health care provider will prescribe this service based on needs after surgery or a hospital stay.

Nursing services include wound care, health monitoring, dressing changes, blood pressure and/or blood sugar monitoring, and many other skilled nursing services, as needed.

Nutritional counseling includes visits from a certified dietician, who will coordinate with other professionals to create a diet appropriate for each patient's needs.

Social work services are provided as needed, including mental health assesment, social services counseling, and other services as needed (?)
