Will my insurance cover my stay?
Medicare and Medicare/Managed Care Insurance cover an eligible patient's stay
for a defined period of time. For those who qualify, Medicaid covers the cost
of long-term care. Patients who require stays not covered by insurance may pay
privately.
What are the steps for admission to long- or short-term care?
Prior to an applicant's consideration for admission, N.Y. State assessment
forms (PRI and Screen), must be completed by a registered nurse. These forms are
reviewed by our interdisciplinary admissions team to determine eligibility for
care. Our finance staff then helps determine the applicant's payment source(s).
Upon availability of an appropriate bed, the eligible patient is admitted.
How should I begin the admission process for my loved one?
If your loved one is currently in...
- A hospital - the hospital discharge planner can fax a PRI
and Screen to us for evaluation at (631-715-2905).
- The community - A PRI and Screen may be completed by one
of our Home Care nurses or a visiting nurse,
and should be faxed to our Admission's Office at (631) 715-2905.
- Another nursing home - you may request that the social worker
fax forms to our Admissions Office at (631-715-2905).
