Discharge Planning

From the first day you are admitted to the Short Term Rehabilitation program at Gurwin, we begin planning your discharge home.  Within the first two weeks of your stay, depending on your personal situation and diagnosis, you will meet with your entire care planning team.  Together, we will map out a path to your return home.
